RESUMO

Objective:To investigate the clinical, neuropsychological, and neuroimage characteristics in patients with corticobasal syndrome (CBS), and to elucidate the exact diagnosis of CBS patients.Methods:Twelve CBS cases admitted to the Department of Neurology, Huashan Hosiptal,Fudan University from April 2019 to July 2021 were retrospectively enrolled in this study. Those data, including clinical features (demographic data and clinical characteristics of cortical dysfunction and movement disorder), neuropsychological assessment [Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) scales score], brain magnetic resonance imaging (MRI) and multi-mode positron emission tomography (PET)/CT, were collected and carefully reviewed. Exact diagnosis of these patients was given according to the disease diagnosis criteria.Results:Cortical dysfunction and asymmetrical movement disorders were found in all cases, with poor response to levodopa. Patients suffered from cognitive impairment (MMSE score 16.16±9.82, MoCA score 13.44±7.35). The cranial MRI demonstrated significant asymmetric atrophy of frontal and parietal lobes, especially in the pre- and post-central gyrus. Fluorodeoxyglucose PET of 12 patients showed asymmetric frontal lobe and basal ganglia (especially caudate and putamen) hypometabolism (obviously on the contralateral side of the affected limb). Tau PET was implemented in 11 patients and displayed that abnormal tau protein deposition was positive in the cortex and/or subcortex in all patients. Of the 4 cases, who completed amyloid PET, amyloid protein deposition was positive in the cortex of 2 patients. As a result, 6 patients were diagnosed as progressive supranuclear palsy, 1 patient was diagnosed as corticobasal degeneration, and 5 patients were diagnosed as Alzheimer′s disease.Conclusions:The etiology of CBS is heterogeneous. The combination of clinical manifestation, cranial MRI and multi-mode PET/CT helps the differential diagnosis of CBS.

RESUMO

Vertical one-and-a-half syndrome (VOHS) is a rare ophthalmoplegia syndrome caused by unilateral thalamo-mesencephalic infarct. It is manifested as a conjugate upgaze palsy and a monocular paresis of downward gaze. Recognition of VOHS helps clinical localization. No cases of VOHS with photos or videos of ocular motility have been reported in China. Herein a case of VOHS caused by unilateral thalamo-mesencephalic infarct is reported, in order to improve awareness about the ocular sign.

Objective To explore the influence of lower concentration of cell free fetal fraction DNA in maternal plasma on non-invasive prenatal test(NIPT) .Methods A total of 3240 pregnant women accepted NIPT in Foshan Maternal and Children′s Hos-pital from April ,2015 to March ,2016 were analyzed retrospectively ,and 150 samples of which were male fetus judged by Z score of Y chromosome and the cell free fetal fraction DNA were lower than 8% were selected .The cell free fetal fraction DNA were in-creased by agarose gel electrophoresis ,then conducted NIPT ,compared with the results of aneuploidy screening .Results The cell free fetal fraction DNA were increased from 5% to 9 .2% by agarose gel electrophoresis .The result of NIPT after increasing fetal fraction was consistent with it before .Conclusion Concentration of cell free fetal fraction DNA has no influence on the result of NIPT when cell free fetal fraction DNA is above 5% .

Objective To assess the correlation between the pregnancy‐associated plasma protein A (PAPP‐A)level with gesta‐tional diabetes mellitus(GDM )in primiparous women .Methods The nonparametric test and Logistic regression analysis were used to retrospectively analyze 168 pregnant women of GDM and 273 normal pregnant women ,including age ,body weight ,PAPP‐A ,freeβ‐human chorionic gonadotropin(fβ‐hCG) .Results The age had significant difference between GDM group and control group(P<0 .01) ,the PAPP‐A concentration and corrected multiple of the median (MoM )of PAPP‐A in the GDM group were significantly lower than those in the control group(P<0 .01) .Conclusion Low level of PAPP‐A during early pregnant stage in primiparas is strongly associated with GDM and can be used as the risk factor of GDM .

Objective To investigate the effects of moving services on negative emotions and quality of life of the patients with leukemia.Methods A total of 128 patients with leukemia were chosen and divided into the control group(n =66)and the intervention group(n =62)according to admission date.The patients of the control group received conventional nursing mode,while the patients of the intervention group were given moving services including updates care philosophy,advocated behavior reflect personalized and multi -style health education.The self -rating depression scale(SDA),self -rating anxiety scale(SAS)before and after the intervention were used to assess changes in patients with negative emotion,while the quality of life scale QLQ -C30 core was used to estimate quality of life in patients before and after treatment.Results After intervention,QLQ -C30 functional scores including physical function, role function,cognitive function,emotional function and social function score were higher than the control group [(68.79 ±13.97)points vs (60.48 ±14.95)points,(53.49 ±15.82)points vs (31.03 ±20.68)points,(88.81 ± 16.46)points vs (73.33 ±16.50)points,(86.48 ±8.21 )points vs (70.30 ±16.43 )points,(31.03 ± 1.27)points vs (26.36 ±8.72)points],and the differences were statistically significant(t =3.094,3.606,3.265, 3.478,3.20,all P <0.05).QLQ -C30 symptom scores including fatigue,pain,nausea,vomiting,dyspnea,insomnia, appetite loss,constipation,diarrhea rating were lower than the control group[(41.81 ±14.25)points vs (52.85 ± 14.57)points,(16.89 ±10.14)points vs (24.43 ±19.05 )points,(17.41 ±13.56 )points vs (29.51 ± 13.64)points,(25.83 ±21.09 )points vs (36.56 ±21.50)points,(11.54 ±8.19 )points vs (25.76 ± 19.07)points,(40.93 ±18.30)points vs (55.87 ±20.05 )points,(15.02 ±10.24)points vs (21.54 ± 12.41)points,(8.66 ±2.41)points vs (15.47 ±9.16)points],and there were statistically significant differences (t =3.179,3.243,3.359,3.470,3.205,3.151,4.282,3.153,all P <0.05).After intervention,the scores of SAS and SDS of the intervention group and the control group were (34.96 ±10.24)points vs (48.24 ±11.71)points, (33.58 ±9.60)points vs (42.06 ±9.62)points,and the differences were statistically significant(t =6.320,5.806, all P <0.05 ).The passing rate of care satisfaction of the intervention group was higher than the control group (98.06% vs 73.08%),and the time of hospitalization of the intervention group was lower than the control group [(22.02 ±2.24)d vs (27.54 ±3.41)d],and the differences were statistically significant(χ2 =4.337,t =3.759,all P <0.05).Conclusion Moving services can improve the quality of life of patients,reduce the negative emotions of patients,improve the quality of nursing services.

AIM:ToinvestigatetheexpressionofFoxp3+regulatoryTcells(Foxp3+Tregs)andprogrammed death receptor 1 (PD1) in gastric cancer tissues and their association with clinicopathological factors and prognosis of the patients.The correlation between the 2 molecules was also analyzed at the same time.METHODS: The tumor sections from 111 gastric cancer patients were stained for Foxp3 and PD1 by the method of immunohistochemistry.The associations of the expression levels of these 2 molecules with clinicopathological factors involved in the disease progression and progno-sis were statistically analyzed .The relationship of their expression was detected.RESULTS:Foxp3 +Tregs and PD1 were expressed in the gastric cancer tissues, and PD1 was expressed in the tumor infiltrating lymphocytes ( TILs) .The expres-sion of Foxp3 and PD1 was correlated with lymph node metastasis, clinicopathological stage and prognosis of gastric cancer patients.The expression of these 2 determinants in the patients with lymph node metastasis and an advanced clinicopatho-logical stage was distinctly higher ( P<0.05 ) .The patients with positive expression of the 2 indexes presented a lower overall survival rate and worse prognosis (P<0.05).A significantly positive correlation between the infiltration of Foxp3 +Tregs and the expression of PD1+TILs was also observed (P<0.01).CONCLUSION:Foxp3 +Tregs and PD1 +TILs co-infiltrate in the gastric cancer tissues, which can be used as biological markers to predict the disease progression and prog-nosis.

ObjectiveTo explore the effectiveness of two ways in the treatment of peripherally inserted central catheter (PICC)-related eczema.Methods Fourty patients with PICC-related eczema were randomly assigned to control group and experiment group.Both were treated by wet dressing with gentamicin and dexamethasone mixture.Differently,the PICCs in the control group were fixed with IV3000 transparent dressing,while those were fixed with silver ions wound dressings in the latter.They were prepared in terms of curative effect and healing time.Result The cure rate in the experiment group was significantly higher than that of the control group (P<0.01) and the healing time was significantly shorter (P<0.01).Conclusion The silver ions wound dressing combined with gentamicin and dexamethasone mixture in the treatment of PICC-related eczema is effective and simple.

The solid interface of different slopes plays a significant role in influencing focus range and image quality of liquid controllable optical lens. In this paper, based on the theoretical model of liquid optical lens, the spherical aberration of the liquid optical lens is calculated. It is found that the spherical aberration is different from the two opposite sides of liquid optical lens, and at the same voltage range, the smaller the cone angle of the solid interface is, the larger the optical aperture of liquid optical lens changes. The spherical aberration of liquid lens can be minimized when a proper voltage is applied and a proper solid interface slope is selected.

Objective To identify lesion patterns and stroke mechanisms in middle cerebral artery (MCA) territory using early diffusion-weighted imaging (DWI) combined with CTA as well as EKG and echocardiography.Methods One hundred and forty-eight acute ischemic stroke patients who had (1) symptomatic lesions located in the unilateral MCA territory on DWI performed within 1 week of symptom onset,and (2) either corresponding MCA disease,internal carotid artery (ICA) disease,MCA & ICA disease or cardio embolism (CE),or (3) neither corresponding MCA disease,ICA disease,nor CE which were taken as group of negative results (NR),were reviewed.Acute DWI lesion patterns were classified as (1) single (small perforator < 2 cm;large perforator ≥2 cm;pial;large territorial;border-zone) and (2) multiple according to principle of single-blind.Results There were 12 types of lesions in MCA territory.Distribution of lesion patterns in different stroke subtypes might be different (χ2= 55.88,P = 0.004).No specific pattern could be found in patients with MCA disease,ICA disease,MCA & ICA disease or CE.Big perforator infarcts might be more common in patients with MCA disease than with ICA disease and CE.Compared with negative group,concomitant perforator and pial infarcts were more common in patients with ICA disease (7/27,χ2=6.61,P <0.05),especially with severe stenosis or occlusion (5/16,χ2=7.32,P < 0.05);No specific pattern could be found in patients with MCA disease or CE.Concomitant perforator,pial,with border-zone infarcts (6/30,χ2= 6.41,P <0.05),and concomitant perforator with border-zone infarcts (4/30,χ2= 5.59,P < 0.05) were more often in patients with severe stenosis or occlusion of MCA.Conclusion Different lesion patterns may indicate different mechanisms of stroke such as hypoperfusion and arterial embolism could be coexistent in MCA territory.The relationship has not been identified perfectly.

<p><b>OBJECTIVE</b>To study glomerular expression of C-C chemokines, monocyte chemoattractant protein-1 (MCP-1), macrophage inflammatory protein-1alpha and beta (MIP-1alpha, MIP-1beta) and the effect of steroid and cyclophosphamide (CTX) intermittent intravenous pulse therapy on expression in patients with crescentic glomerulonephritis (CGN) to further investigate the underlying mechanism of the treatment.</p><p><b>METHODS</b>Twelve patients with initial biopsy-proven CGN(2), 6 with lupus nephritis (lupus-CGN, LN-CGN) and 6 with vasculitis, (vasculitis-CGN, V-CGN) were enrolled in this study. They underwent an initial biopsy before steroid and CTX intermittent intravenous pulse therapy and were biopsied again one to three months later. Expression of MCP-1, MIP-1alpha, MIP-1beta, and CD68 in glomeruli with cellular and fibrocellular crescents were examined by immunohistochemical analysis in serial sections of renal biopsies. The effect of the pulse therapy on histopathological changes was also observed.</p><p><b>RESULTS</b>Although steroid and CTX intermittent intravenous pulse therapy markedly reduced the degree of glomerular crescent formation both in LN-CGN and V-CGN, the effect of the therapy on glomerular chemokine expression was significantly different between LN-CGN and V-CGN. It was found that steroid and CTX intermittent intravenous pulse therapy reduced the expression of CD68, MCP-1, and MIP-1alpha, but had no effect on MIP-1beta in glomeruli with cellular crescents of patients with LN-CGN. In patients with V-CGN, the therapy also reduced the expression of CD68, but had no effect on MCP-1, MIP-1alpha, and MIP-1beta in glomeruli with cellular crescents. It was noted that the degree of glomerulosclerosis and tubular interstitial fibrosis increased more significantly at the second biopsy in V-CGN as compared to LN-CGN.</p><p><b>CONCLUSIONS</b>The efficacy of steroid and CTX intermittent intravenous pulse therapy in CGN might be affected by reduction of glomerular chemokine expression. The different changes in glomerular expression of MCP-1 and MIP-1alpha in patients with LN-CGN and V-CGN after pulse therapy may correlate to different responses to treatment and prognosis.</p>

RESUMO

AIM To study the protection of 6,7 dimethexycoumarin on hypoxic animals. METHODS The effect of 6,7 dimethexycoumarin on survival time and survival rate of mice subjected to hypobaric, normobaric and chemical hypoxia were observed and the oxygen consumption of isolated tissues were measured. RESULTS 6,7 dimethexycoumarin obviously prolonged the survival time and promoted the survival rate of hypoxic mice, increased the mouth opening time of isolated mice and reduced the oxygen consumption of isolated cardiac muscle of mice. This contributes to exploring the mechanism of antiradiation and poison reducing of 6,7 dimethexycoumarin. CONCLUSION 6,7 dimethexycoumarin shows remarkable effect on tolerance to hypoxia in mice.
